Finding reason in delusion

Dementia -- an acute loss of cognitive ability -- can be marked by memory loss, decreased attention span, and disorientation. It occurs in severe disorders such as Alzheimer's disease. Despite the fact that the condition is common, especially among older persons, there is still a lack of effective treatment.

According to Prof. Jiska Cohen-Mansfield of Tel Aviv University's Herczeg Institute on Aging and Sackler Faculty of Medicine, dementia sufferers are often prescribed psychotropic drugs to mitigate symptoms such as delusions. But this tactic can cause more harm than good, she says. Many of the delusions experienced by dementia patients may have a rational basis and could be more effectively treated through behavioral therapy than by medications, suggests Prof.Cohen-Mansfield. A better understanding of delusions has direct implications for the care and perception of those who suffer from dementia.

The study, done in collaboration with Prof. Hava Golander of the Department of Nursing and Drs. Joshua Ben-Israel and Doron Garfinkel of the Shoham Medical Center, was published in the journal Psychiatry Research.

A rational basis for delusion

The researchers examined six common categories of delusion, including fears of abandonment, suspicions that patients' possessions were being stolen, and feelings that they were not "at home." The participants in the study were 74 adults from nine nursing homes in Israel who had received a diagnosis of dementia. The research team questioned caretakers, including registered nurses and nursing home staff, who had daily interactions with the patients.

Researchers assessed elements including the patient's mental state, behavioral pathology, and incidents of past trauma. Caretakers were asked to describe not only the patient's delusions, but to explain the circumstances under which they emerged. Taking into account all these parameters, researchers discovered that a large percentage of the delusions that the caregivers described seemed to have logical explanations. Some were even the result of the patient re-experiencing traumas they had suffered earlier in their lives.

"If you begin to think about these delusions from the point of view of the dementia patient, you begin to understand that their delusions are explainable reflections of the reality they live in," explains Prof. Cohen-Mansfield. For example, for patients who felt that they were not at "home," the nursing home did not satisfy their definition of home. Anxiety often accompanied separation from the outside environment or from their loved ones ? a rational response.

A better quality of care

These findings can have an impact on the way heath care providers and family members respond to dementia patients, says Prof. Cohen-Mansfield. Characterizing their delusions as "psychotic" puts them in the category of severe mental illness, which is frequently inappropriate. Instead, caretakers can devise methods to work with patients that take context into account.

Prof. Cohen-Mansfield also notes that participants in the study were highly medicated, with 47 percent on antidepressants, a third taking sedatives/hypnotics, and 13.5 percent on antipsychotic medications. "If you can ascertain why the patient is experiencing these 'delusions,' you can create another treatment plan that addresses issues of orientation," she explains.

In people with dementia, delusions do not really match the psychiatric definition of psychosis. A closer analysis of these behaviors is likely to promote empathy, understanding, and ultimately a more humane and compassionate treatment, the researchers concluded.

LOS ANGELES (AP) ? The feast is on: "The Hunger Games" has taken in $68.25 million domestically in its first day, a record for a non-sequel.

The Friday total for Lionsgate's "The Hunger Games" was the fifth-best opening day ever and puts the movie on track for the best debut weekend ever in March. That record is held by "Alice in Wonderland" with $116.1 million.

The big start should translate into an opening weekend of as much as $140 million domestically, according to Paul Dergarabedian, analyst for box-office tracker Hollywood.com.

First-day revenues for "The Hunger Games" were well behind the record $91.1 million for last summer's "Harry Potter and the Deathly Hallows: Part 2." But "The Hunger Games" came within a few million dollars of each of the last three "Twilight" movies, whose opening days ranged from $68.5 million to $72.7 million.

"The Hunger Games" stars Jennifer Lawrence as a teen forced to compete in a televised death match against other youths in a future North American society where a privileged capital city oppresses the people of 12 outlying worker districts. The film is based on the first book in a best-selling trilogy by author Suzanne Collins.

It has also angered North American customers who view outsourcing what used to be American jobs as borderline traitorous, especially with the economic situation they are in.

We foresee this trend reversing for two reasons. Firstly, companies are feeling the negative impact outsourcing has on their customer service. While they?re able to cut costs, the quality of customer service takes a nosedive. Acquiring new customers to replace those lost from poor service is a costly and daunting endeavor.

For example, in December 2011, as a result of an agreement with its unions, US Airways announced it is bringing 400 customer service jobs previously outsourced to Manila back to centers in North Carolina. AT&T recently decided to bring back 5,000 customer service jobs back to the U.S as a result of its merger with T-Mobile.

Furthermore, in an effort to encourage insourcing, an anti-outsourcing bill targeting call centers was recently introduced in congress. This trend should be a win for customers, the economy, and businesses who implement it correctly.

7. Voice of Customer (VOC)

More companies are implementing VOC programs for their proven ability to positively impact the customer experience. Companies collect feedback to help them make improvements across their organization. The growth in VOC is driven by its benefits such as better customer loyalty and retention.

Despite the increased adoption of VOC, many organizations are still not reaping its full value. Aligning the right people and processes for maximum impact remains a challenge. Taking action on the customer feedback that is collected is another hurdle most companies face. As the VOC trend increases, getting it right remains a challenge.

8. Video

We predict video will be increasingly used to provide customer service and support in 2012. With approximately 70% of global online consumers watching online video, consumers have an alternative way to stay informed.

A well put-together video makes information easier and faster to understand and share. This is why people first flock to video tutorials before reading a manual, asking help on forums, or reaching out to an agent. Talking To Aging Parents About Money | The Law Office of Michael ...

For an aging parent, the change from being independent and caring for others, to becoming someone who relies on others on a daily basis, is often fraught with highly charged emotions and difficult transitions. When you add the difficult topic of money to the mix, bringing up financial discussions with your aging parent can sometimes feel almost impossible. Let?s look at several tips you can use in order to have a positive discussion about the topic.

Tip 1: Set out your desires. When you confront an aging parent about money, parents can often feel cornered or accused, causing them to shut down or be defensive. Take some time to sit down and reflect on why you want to talk about finances and what your concerns are about your parents. Write these down so you can later bring them up and couch the discussion in terms of your concern.

Tip 2: Ask, don?t order. The loss of independence that elderly people face is often more daunting than any medical condition or other change in lifestyle. When you bring up financial concerns, it?s important to raise them as questions and not appear as if you are giving parents an order or making demands.

Tip 3: Recognize the parent?s feelings. Often, a simple phrase such as ?it must be difficult to think that you have to rely on your children after so many years of looking after us? can establish empathy and sympathy with your parent.? As long as your parents understand that you are considering his or her feelings, this can go a long way to making the discussions easier.
